REGISTRATION 
 
  Print and fill out the forms below.  Enclose a check for a minimum of $50.00, however registration fees must be paid in full along with any previous fundraising responsibilities, prior to the start of team practices in mid October.

MANDATORY  USA Hockey online Registration for youth players/coaches. It’s easy and only takes a few minutes to complete.
 
High School players do not need to register with USA Hockey, or fill out any USA Hockey forms.
 
Step 1. Go to www.usahockey.com, select “Players” then “Online Registration” and follow the steps until you have printed a confirmation page.  Repeat this process for each player/coach.
 
Make sure and register your players name as it is listed on their birth certificate. 
 
A credit card will be required to pay the $40 registration fee for each player 7yrs and older and all youth coaches.  This fee will be in addition to the PRAHA registration fee. 
 
Step 2. Bring the confirmation page (and birth certificate for new players to the association ) for each child to Registration Day, Sept 16th 5-8pm, at the arena. 
 
Step3. Fill out forms posted below. 
Youth players: forms 1, 3 & 4. 
High School players: forms 2 & 4. 
Coaches: form 3
  Fill them out ahead of time to expedite registration at the arena.

4). Fundraising / Responsibilities form
 
A minimum of $50.00 will be required per child, with the balance due prior to the start of team practices.
